QUICK ANSWER
Quick answer
Sinistral affects Prefixes, Dextral affects Suffixes; the rest of the name tells you which currency.
Only one non-crafting (survival/league) Omen can be active at a time.
Start with Omen of Sinistral/Dextral Exaltation and an Omen of Crystallisation.
Add Omen of Greater Exaltation, Homogenising Exaltation and Whittling for deterministic crafts.
Leaving an expensive Omen active when you run a different currency by mistake.

ITEM FAMILY
Omen families
Omens split into crafting control Omens and non-crafting (survival / league) Omens. Within crafting Omens, the Sinistral/Dextral prefix tells you which modifier side is affected.
Exalted Orb
Sinistral / Dextral / Greater Exaltation
Prefix / Suffix / two modsSinistral adds only Prefix modifiers, Dextral only Suffix modifiers. Greater Exaltation adds two random modifiers, and stacks with a Sinistral/Dextral Omen to add two of the chosen side.
Orb of Annulment
Sinistral / Dextral Annulment
Remove Prefix / SuffixLimits which side the Orb of Annulment can strip a modifier from.
Chaos Orb
Sinistral / Dextral Erasure
Chaos rerolls one sideForces a Chaos Orb to reroll only the Prefix or only the Suffix block.
Perfect / Corrupted Essence
Sinistral / Dextral Crystallisation
Essence removes one sideYour next Perfect or Corrupted Essence removes only Prefix or only Suffix modifiers - the safe way to use essences.
Exalted / Regal
Homogenising Exaltation / Coronation
Same-tag modifiersThe next Exalted/Regal Orb can only add a modifier that shares a Tag with an existing modifier on the item.
Chaos Orb
Whittling
Remove lowest-tier modYour next Chaos Orb removes the lowest-tier modifier instead of a random one. High-value on trade leagues.
Exalted Orb
Catalysing Exaltation
Spend Catalyst QualityThe next Exalted Orb consumes all Catalyst Quality to bias toward that modifier type. See the Catalysts guide.
Non-crafting
Amelioration / Refreshment / Resurgence
SurvivalReduce XP loss on death, refill flask/charm charges at Low Life, or restore Life/Mana/ES at Low Life. Only one non-crafting Omen active at a time.
EFFECT
How an Omen changes a currency
An Omen does not change the item directly. It rewrites the rules of the next currency that matches its condition. For example, with Omen of Dextral Exaltation active, your next Exalted Orb adds only Suffix modifiers instead of a random modifier.
Several Omens can be active at once and their effects stack, as long as they are compatible. Incompatible Omens (such as two that both want to be the 'next Exalted' controller) will deactivate the earlier one when you activate the later.
- Active Omens show a red border in the inventory.
- An Omen is spent only when its matching action occurs.
- If you never perform the matching action, the Omen is not consumed and stays in your inventory.
VALID TARGET MATRIX
What triggers an Omen
Each Omen names the exact currency and modifier side it watches for. It does nothing for any other action.
| Target | Exalted Orb | Orb of Annulment | Perfect / Corrupted Essence | Note |
|---|---|---|---|---|
| Omen of Sinistral Exaltation | Yes | No | No | Adds only Prefix modifiers |
| Omen of Sinistral Annulment | No | Yes | No | Removes only Prefix modifiers |
| Omen of Sinistral Crystallisation | No | No | Yes | Essence removes only Prefix modifiers |
ACQUISITION
How to get Omens
Omens are an endgame crafting resource obtained mainly from one mechanic, with a couple of extra routes.
Ritual altars
The primary source. Spend Tribute at a Ritual Altar for a chance at Omens; some are extremely valuable.
Abyss mechanic
Abyss Omens drop from the Abyss content and follow the same activation rules.
Currency Exchange
In trade leagues you can exchange currency for many common Omens directly.
USE FLOW
How to use an Omen
Choose the Omen
Pick the Omen that matches the currency and side you want to control.
Activate it
Right-click in your inventory until the red border appears.
Use the currency
Perform the matching currency action; the Omen is consumed and its rule applies.
PRE-USE CHECKLIST
Pre-use checklist
| Done | Check | Why it matters | Quick fix |
|---|---|---|---|
| Omen matches the currency Confirm the Omen names the exact currency you are about to use. | A mismatched Omen will not trigger and you waste the currency. | Re-read the Omen name before the currency. | |
| Side is intended Sinistral = Prefix, Dextral = Suffix; confirm which you need. | Wrong side wastes an expensive Omen. | Swap to the correct Sinistral/Dextral Omen. | |
| No incompatible Omen active Check no earlier Omen of the same currency is already active. | An incompatible Omen deactivates the earlier one. | Activate the right one last. | |
| Item is ready The target item has the modifiers you expect before the currency. | The Omen changes one action irreversibly. | Finish other prep first. |
OUTCOME ANALYSIS
Outcomes by Omen + currency
| Scenario | Benefit | Risk | Audience | Verdict |
|---|---|---|---|---|
| Dextral Exaltation on a 1-suffix Rare | Fills the Suffix block safely | Low | Targeted crafters | Low Great default for finishing a Rare |
| Sinistral Crystallisation + Perfect Essence | Keeps Suffix mods, swaps a Prefix | Medium | Essence crafters | Medium The safe way to Perfect-craft |
| Sinistral Annulment on a 1-prefix Rare | Removes only the bad Prefix | Low | Fixers | Low Cleaner than a plain Annulment |
CRAFTING
Crafting combos that work
The most common deterministic combo is Omen of Dextral Exaltation plus Omen of Greater Exaltation: the next Perfect Exalted Orb adds two Suffix modifiers at a minimum tier. Stacking a Sinistral and Dextral Omen with a Greater Omen lets you fully control both sides.
For essences, pair Omen of Sinistral or Dextral Crystallisation with a Perfect/Corrupted essence so you only ever lose the side you do not care about. Homogenising Exaltation is an advanced option: it restricts the next Exalted/Regal to modifiers sharing a Tag with what is already on the item.
- Dextral Exaltation + Greater Exaltation = two Suffix mods from one Perfect Exalted.
- Sinistral Crystallisation + Perfect Essence = safe Prefix swap.
- Homogenising Exaltation needs a good understanding of modifier Tags.
RISK ANALYSIS
Risk analysis
| Scenario | Gain | Loss | Risk | Recommendation |
|---|---|---|---|---|
| Expensive Omen left active by mistake | None | Wasted Omen on the wrong currency | High | Activate the Omen only right before the matching currency. |
| Two same-currency Omens active | None | Earlier Omen silently deactivated | Medium | Keep only one controller Omen per currency active. |
| Whittling on a finished item | Removes only the lowest-tier mod | Still removes a mod you might want | Low | Use only when the lowest mod is unwanted. |
USE / SELL / HOLD
Use, sell or hold
Worth it when the craft's value clearly exceeds the Omen's price.
COMMUNITY EVIDENCE
Community evidence
Why did my expensive Omen vanish without doing anything?
A player activated an Omen, then used a different currency and lost it.
An Omen is consumed only by its matching action; using a non-matching currency does not spend it. The loss was likely an incompatible Omen deactivating, or using the wrong currency after activation.
Activate the Omen immediately before the exact matching currency; keep only one controller per currency.
Does the Omen only stop deletion, or also control what gets added?
Players confused whether Crystallisation/Exaltation Omens also gate additions.
Exaltation Omens control what is added (Prefix/Suffix only). Crystallisation/Annulment/Erasure Omens control what is removed.
Match the Omen to the action: Exaltation = addition side, Annulment/Erasure/Crystallisation = removal side.
PROBLEMS & DIRECT ANSWERS
Problems and direct answers
Omen did not trigger
Direct answer: The currency you used was not the one the Omen watches for.
- Re-read the Omen name; confirm the exact currency.
Wrong side was affected
Direct answer: You used a Sinistral when you needed Dextral, or vice versa.
- Sinistral = Prefix, Dextral = Suffix; swap before the next use.
Omen disappeared unexpectedly
Direct answer: An incompatible Omen of the same currency deactivated it when you activated the newer one.
- Keep a single controller Omen per currency active.
PATCH HISTORY
Patch history
Omens remain inventory-activated crafting controllers; Sinistral/Dextral naming and the Crystallisation/Exaltation/Annulment/Erasure families are unchanged in 0.5.4.
