Lythara, the Wayward Spear is a high-pressure Interlude boss fought in a small arena. The fight is continuous but escalates with threshold-based Wildwood Wisp empowerments rather than discrete phases.
The defining threat is area denial: poison pools and lightning / chaos storms linger, and her spear throws spawn storms that keep firing projectiles. Positioning is the whole fight.

Arena State and Ground Hazards
The arena starts clean, then Lythara lays down ground hazard zones that persist for a long time. Thunder Spear and Chaos Throw leave lightning / chaos storms that periodically fire additional projectiles.
As her HP drops, Wildwood Wisps (Wild / Vivid / Primal) spawn; if she touches one she is empowered (e.g. Frenzied Boar stampede, Raging Ox). Guides advise fighting her away from wisps and resetting at the checkpoint if she gets 2+ buffs.

Kill Lythara; the Gemcrust Skull drops on the ground. Using it also grants a Level 14 Uncut Spirit Gem.
Pick it up — and watch a full backpack, which can make the Skull vanish on drop. Then RIGHT-CLICK it in inventory to apply +40 maximum Spirit. If you already earned it on a pre-0.3.0 character, it will not drop again.

Lythara punishes any build that must stand still. The fight is a positioning puzzle around persistent storms, so mobility and burst beat sustained DPS.
Ranged kits can kite the storms; melee must use the clean-ground windows between throws. Avoid the Wildwood Wisps entirely — an empowered Lythara is a different, harder fight.

Lythara not dropping the Gemcrust Skull / marked done
Players killed her several times with no Skull; one cause is a pre-0.3.0 character that already earned the +40 from Act 3 Cruel, so the map shows it done and no new Skull drops.
The Skull is a one-time campaign drop. A full backpack at drop can also swallow it (one player found it in 'extra storage').
Pick up the Gemcrust Skull after the kill; if earned pre-0.3.0 it will not drop again.
Lythara boss fight: no Spirit reward, no completion
Multiple players: 'fought 3 times, no reward'; 'inventory was full and the reward disappeared'. One resolved by finding it in extra storage.
