How the life-to-Energy-Shield conversion works
Ghostwrithe's first modifier reads '100% of Maximum Life is from Energy Shield'. This does not reduce damage; it relocates your health. Your Maximum Life value is added directly into your Maximum Energy Shield. The net effect is a single defensive pool: Maximum Energy Shield = (Maximum Energy Shield from all other sources) + (Maximum Life from all sources), and Maximum Life becomes 0.
Worked example: a character whose tree and gear give 2500 Maximum Life, whose body, tree, gear and auras give 3000 Energy Shield, and who then equips a Ghostwrithe. After conversion the character has 2500 + 3000 = 5500 Maximum Energy Shield and 0 Maximum Life. Every point you would have spent on life now just makes the ES pool bigger, because life is folded into it.
The second modifier, '(20-30)% of Hit Damage is taken from Energy Shield before Life', is mostly redundant once life is 0, but it is the explicit rule that keeps damage on Energy Shield. In normal PoE2 a hit depletes ES first and overflows into life; Ghostwrithe guarantees that an extra 20-30% of each hit is routed to ES before life is ever touched. Since life is already 0, the practical message is simply: all your incoming hit damage lands on your rechargeable Energy Shield.
The third modifier is the catch: 'Cannot Leech or Regenerate Life, and Cannot gain Life from Flasks'. With no life pool, life flasks heal nothing, life leech returns nothing, and life regeneration ticks on an empty bar. You are fully dependent on Energy Shield sustain. This is the real cost of the item, and it is why Ghostwrithe only pays off on characters that already invest in ES sustain (recharge rate, ES leech, ES-on-block, ES regeneration).
Why players run it anyway: folding life into ES gives you one very large pool instead of two smaller ones, and that pool benefits from Energy Shield recharge, which is often faster and more reliable than life flask uptime once you invest in 'Energy Shield Recharge Rate' and 'reduced ES start-of-recharge delay'. It also makes you Chaos-Inoculation-adjacent: with 0 life, chaos damage cannot kill you through life, so you only need to protect a single ES bar from all damage types.

Ghostwrithe is for characters that already live on Energy Shield and want to stop splitting defense between life and ES. The following are the realistic homes; each assumes you have an ES sustain plan.
Stormweaver Archmage
A natural home: Archmage scales mana into both damage and ES, so the converted life just feeds the same giant pool. Pair with ES recharge scaling and a source of ES leech so the single bar stays topped up.
↗Chaos-Inoculation-style Invoker monk
Monks can stack ES and convert life into the same bar, gaining CI-adjacent immortality (chaos cannot kill through life) while keeping their life passives working for ES. Requires disciplined ES recharge and leech.
↗Low-life Energy Shield Templar
Players who want a low-life fantasy without Pain Attunement complications use Ghostwrithe to merge pools and run a huge ES bar fed by auras and Corsair's flasks (which still work on ES).

Alternatives: normal ES chest, CI keystone, life armour
Ghostwrithe is not a strict upgrade over a well-rolled rare Vaal Regalia. A rare ES chest can give a higher base Energy Shield plus useful suffixes (ES%, reduced ES start delay, attribute requirements) without forcing you to abandon life sustain. Use a rare chest when you still want a life buffer.
Compared with the Chaos Inoculation keystone, Ghostwrithe achieves a similar 'immortal on ES, chaos-safe-on-life' result but without setting Maximum Life to 1. With CI your life is 1 and you must still build ES; with Ghostwrithe your life is 0 and your life passives instead inflate ES. CI is cleaner and does not disable life leech/regen on a separate bar, but Ghostwrithe lets life-passive-heavy trees still pay off as ES.
Against a normal life-based body armour, Ghostwrithe is strictly worse for any build that relies on life flasks or life leech - which is most attack builds. Those should never consider it. The decision is really 'go all-in on one ES pool' (Ghostwrithe) versus 'keep life + ES separate' (rare chest or CI).

How to get Ghostwrithe
Ghostwrithe is a relatively common unique body armour, so the realistic path is trade rather than farming.
Random drop (any level, Act 1 onward)
It can drop from any monster once you reach the required level. You cannot target it; raising unique drop rate via 'Increased Rarity of Items Found' and juiced Waystones increases how often uniques appear, but this is slow for a specific body armour.
Orb of Chance on a white Vaal Regalia
Using an Orb of Chance on a normal Vaal Regalia can yield Ghostwrithe, but the chance is low and the currency is better spent elsewhere. Most players skip this route.
Player trade (recommended)
Buying on trade is the reliable way to get a specific base-ES roll. Because it is common, even a high base-ES roll is affordable. Shop for the highest base Energy Shield you can afford; the (20-30)% suffix roll barely matters. Prices vary by league and region - check the live trade site.

Yes. Once Maximum Life is folded into Energy Shield, your life pool is 0, so life flasks, life leech and life regeneration have nothing to act on. The third modifier makes this explicit. You must rebuild sustain on Energy Shield: recharge rate, ES leech, ES-on-block, ES regeneration.
Replace life sustain with ES sustain. Stack 'Energy Shield Recharge Rate', cut the ES start-of-recharge delay, and grab an ES leech source (e.g., Ghostshroud or an ES-leech support). Life flasks become decorative.
Is Ghostwrithe basically a free Chaos Inoculation without the keystone?

Ghostwrithe is a unique Vaal Regalia with: 100% of Maximum Life is from Energy Shield; (20-30)% of Hit Damage is taken from Energy Shield before Life; Cannot Leech or Regenerate Life, and Cannot gain Life from Flasks. This is the only current version.
Ghostwrithe entered Path of Exile 2 Early Access as a conversion-body unique. The core three modifiers have been stable since introduction; verify exact roll tiers in-game as balance passes occur.
Low-level uniques became Runeforgeable. Ghostwrithe (level 1) qualifies: the three modifiers are retained and the Vaal Regalia base gains higher Energy Shield plus a Runeforged base.
The suffix roll is a fixed range, so a Divine Orb does not change the (20-30)% tier. A Vaal Orb corruption can add an ES% or ES-recharge implicit but consumes a unique - corrupt a replaceable copy.
