Quick answer

Take an Inscribed Ultimatum to the Temple of Chaos to start a Trial of Chaos. Its Area Level sets the room count: 39-59 = 4 Trials, 60-74 = 7 Trials, 75-80 = 10 Trials. Only the 10-Trial version drops a Fate. Collect all three Fates - Deadly (Bahlak, the Sky Seer), Victorious (Chetza, the Feathered Plague) and Cowardly (Uxmal, the Beastlord) - then clear another 10-Trial run and insert the three Fates into the door in the final reward room to fight the Trialmaster.

The area-level thresholds above are the current 0.5 values published on the wiki; confirm them against your own client when 0.5.4 lands, as GGG can shift breakpoints between patches (current 0.5, pending in-game verification).

You do NOT need the three Fates to run a Trial of Chaos for points 1-6; you only need them to spawn and open the Trialmaster door for points 7-8.

Use for

Entering the Trial of Chaos for Ascendancy points, and (with all three Fates) challenging the Trialmaster for your final two points.

Check first

Read the Ultimatum's Area Level. If it is below 75 it will only give 4 or 7 Trials and will NOT drop a Fate.

Avoid

Wasting a high-level Ultimatum by failing the run - failing forfeits the attempt, and you must start a new Ultimatum.

The Trial of Chaos access family has one entry key (the Inscribed Ultimatum) and three end-game keys (the Fates). You farm the Ultimatum from anywhere in the endgame, and you farm the Fates only from 10-Trial runs. All three Fates are required together to open the Trialmaster's door.

ACQUISITION

How to get Inscribed Ultimatums and Fates

The entry key drops almost anywhere; the Fates are gated behind the highest-tier Ultimatums. There is no deterministic Fate vendor - you either farm 10-Trial runs or trade for the missing one.

  1. Monster drops (Ultimatum)

    Inscribed Ultimatums drop from monsters from Act 3 onward and throughout the endgame, with higher Area Levels appearing in higher-tier content.

  2. 10-Trial runs (Fates)

    Only an Area Level 75-80 Ultimatum runs 10 Trials and fights a Fate boss on round 10. Each run drops ONE random Fate, so you need several runs (or trades) to collect all three.

  3. Currency Exchange / trade

    Because Ultimatums and Fates are tradeable, you can buy a specific missing Fate from other players instead of re-farming the matching boss.

  4. Corrupted magic Ultimatums

    The Trialmaster can drop corrupted magic Inscribed Ultimatums with two affixes (a reward prefix and a dangerous area suffix). These are a higher-risk re-run, not a separate acquisition path.

USE FLOW

How to use them

  1. Open the Trial of Chaos

    Take the Inscribed Ultimatum to the Temple of Chaos and use it at the altar. The Ultimatum is consumed and a run of 4, 7 or 10 Trials begins.

  2. Clear to round 10 (for Fates)

    If your Ultimatum is Area Level 75-80, survive all 10 rounds. The round-10 boss is one of the three Fate bosses and drops a single random Fate.

  3. Collect all three Fates

    Repeat 10-Trial runs (or buy the missing ones) until you hold Deadly, Victorious and Cowardly Fate at the same time.

  4. Open the Trialmaster door

    Clear another 10-Trial Ultimatum while holding all three Fates. A door appears in the final reward room near the Ascension Shrine - insert the three Fates to enter the Trialmaster's Tower.

Before

You hold an Inscribed Ultimatum (any level) and run the Trial of Chaos for Ascendancy points 1-6.

After

You hold all three Fates and clear a 10-Trial run, opening the Trialmaster door for points 7-8 and his unique loot.

Points 1-6 need only the Ultimatum; points 7-8 need the three Fates and the door.
