A three-notable jewel where two notables are strong for your build and one is filler is still an excellent buy - you reclaim two notables' worth of points plus a real stat spread.
OVERVIEW
Overview and current versus legacy
Megalomaniac collapses the cost of reaching good Notables into a single socket. Socket it into an allocated jewel socket and it immediately grants the effects of whatever 2-3 Notables it rolled, exactly as if you had walked to each and spent the points - except the points stay in your pool to spend elsewhere. Notables are the large, named passives with icons and multi-line text; the jewel effectively anoints two or three of them at once, for free.
Because the notables are random and the jewel is Corrupted on drop, Megalomaniac is a lottery, not a crafting target. Most drops are junk for any given build, but a jewel that is junk for you can be perfect for someone else, which is why its trade market is huge. Value follows the notable value model: net value equals the effective notables gained, plus the value of pathing saved, minus one jewel socket, minus the pathing to the socket, minus the Rare/Unique jewel you gave up.
Current versus legacy: since patch 0.5.0 Megalomaniac drops identified, so you see the rolled Notables immediately instead of needing to identify it. The allocation behaviour (2-3 random Notables, no duplicates) is unchanged from earlier in Early Access. The Simulacrum remains the only source.

Decide which 2-3 Notables would save you the most pathing on your current tree. Deep, build-defining Notables are worth the most.
Socket Megalomaniac in an allocated slot
Place it in any allocated jewel socket. The rolled Notables are granted instantly and the saved points return to your pool.
Spend the reclaimed points
Use the freed points to deepen another cluster or fix a defensive gap - that is the real payoff.
Search the trade site for specific Notables
On the official trade site: set Item Category to Jewel, Rarity to Unique, and type the exact name 'Megalomaniac'. Add a Stat Filter for 'Allocates Passive Skill' and repeat it for a second (and third) Notable you want. Set League to the current league and toggle Online/Offline as needed.
Read full notable descriptions before buying
The in-game client shows only notable names; open the trade listing in a web browser to see full descriptions on hover, or check PoE2DB / PoE2Wiki for the exact notable. Never buy on the jewel title alone.
Reaching three deep Notables might cost 15-60 passive points of pathing you do not have.
Megalomaniac grants those same Notables from one socket and returns the points to your pool.

Notables granting crit multi or a damage-type multiplier your build already scales are worth far more than flat attribute notables.

Alternatives
If you cannot find a matching roll, a Rare Diamond Jewel or another Diamond Unique may serve better. From Nothing grants a random Keystone (useful when a single Keystone is the goal), while Against the Darkness reshapes the small passives around its socket for attribute or defence stacking.
Anointment on an amulet grants one Notable at a time and is fully chooseable, so for a single critical Notable an anointment is more reliable than gambling on Megalomaniac's random roll.
Timeless Jewels and Cluster-style setups are complementary ways to reshape the tree, but none replicate Megalomaniac's free 2-3 Notable allocation.
