Overview, ring-size visual and current versus legacy
Controlled Metamorphosis is the only Unique Jewel whose allocation zone is a ring (an annulus) rather than a filled circle. When socketed into an allocated Jewel Socket, it draws two concentric rings: an inner ring and an outer ring. Any passive node that falls on the band between those two rings - the annulus - can be allocated without being connected to your tree. Crucially, the space inside the inner ring is dead: nodes there cannot be taken through the jewel, which is the single biggest surprise for new users who expect a normal circular radius.
Picture the passive tree as a grid of nodes. Place the Jewel Socket at the centre. Draw a small circle of radius r1 (the inner ring) and a larger circle of radius r2 (the outer ring) around it. The annulus is the donut-shaped band between r1 and r2. Nodes sitting on that donut are reachable; nodes strictly inside r1 and nodes outside r2 are not. Because the band is thin, the ring size roll (1-8) decides how many useful nodes land on it - which is why a bigger ring is not automatically better: it depends on whether your target nodes actually sit on the band at that size.
The jewel also applies -(20-5)% to all Elemental Resistances. This penalty is independent of the ring size: every copy, from Very Small to Massive, can roll anywhere in that range, and a Massive ring with a -5% roll is the most valuable combination. You still pay the passive point for every node you allocate; the jewel only removes the pathing requirement, so the saving is the pathing points you skip.
Current versus legacy: before patch 0.4.0, Controlled Metamorphosis also applied -(23-3)% to Chaos Resistance. That Chaos line was removed in 0.4.0 and does not return on new 0.5.4 drops - only the Elemental penalty remains. Legacy copies dropped before 0.4.0 keep their Chaos penalty. There is also a documented edge case where a corrupted roll of 9 or higher (outside the 1-8 range) makes the ring-size modifier disappear and the jewel forms no ring; treat any such copy as a bug-affected outlier and trade only for valid 1-8 rolls.

A normal or Rare Diamond Jewel gives flat stats for the same socket with no resistance cost and no ring-size gamble. If your target nodes are cheap to path to, a Rare jewel is usually the safer value.
From Nothing is the closest cousin: it also remote-allocates, but through a fixed disc around a bound Keystone and with no resistance penalty - at the cost of being Keystone-locked. Megalomaniac grants random Notables directly. Against the Darkness boosts small passives in a radius. Pick by what your tree actually needs.
Because Controlled Metamorphosis costs Elemental Resistance suffixes, the real alternative is often just pathing normally and using the socket for a Rare jewel or a resistance-friendly unique - only commit when the skipped pathing clearly beats the suffix loss.

Why can't I allocate a node that looks inside the radius?
A player expected a filled circular radius and could not take a node in the centre.
Controlled Metamorphosis forms an annulus, not a disc. Only the band between the inner and outer ring allocates; the inner circle is dead space. A node that looks 'inside' may sit in that dead centre.
